Please submit completed forms to Pruet Production Co. division order department.

Affidavit of Death and Heirship

Please allow 10 days for the P.O. to process your check.

If after 10 days you have not received your check / or if the check has been misplaced, please call our office for a stop payment (provide:  name, owner number, social security number)

You may also submit a Lost Check Form.

A copy of Marriage Certificate / or other legal recorded documentation indicating name change must accompany the Name Change Form.
